Poor Culture
If an organisation has a poor culture, then it developed it over time. Cultures are not instantly made, or instantly changed. Culture can be destroyed instantly, but not made into something else - unless developed
A seriously flawed culture may need destruction in order to allow another to develop unencumbered.
It is pretty well impossible to design and implement a culture. All you can do is set up the environment in which the desired culture can develop, and start it going.
The easiest way to establish whether there is a flawed culture is to test climate. Culture audits require specialist skills and are ambiguous by nature. Climate is far better understood and agreed upon - and easier to measure.
